Celebrating our achievements

As part of our work, we’ve been looking at a wide variety of services and, importantly, talking to the public and to clinicians about how we can do things differently. We wanted to share just a few of the stories behind the changes we’ve made.

Some of our achievements so far:

A significant number of carers support payments have now been paid and these have been very much appreciated by the carers who have received them.

A consultation was held to ensure the right mix of short breaks is being offered to disabled children, young people and their families.

There are many types of short breaks available for disabled children and young people including overnight short breaks. The consultation was held because Preston Skreens, a centre for disabled children and young people in Medway and Swale go for overnight breaks, was due to close.

An integrated discharge team has been set up to help patients leave Medway hospital when they no longer need acute inpatient care.

Medway CCG is working with partners such as Heart UK to bring new technology into GP practices.

Audit + is a software package that works with GPs Clinical systems to allow patient specific prompts to be added so that the GP or nurse can better tailor the care they provide to the individual.
